Privacy
How Hope Filled Horizons Pty Ltd handles personal information, including enquiries made through this website. Last updated 13 October 2026.
Who we are
Hope Filled Horizons Pty Ltd (ABN 59 678 759 905) is a Queensland disability support provider supporting Ipswich, Brisbane, Gold Coast and surrounding communities. For privacy questions, access or correction requests and complaints, use the contact form (choose “Something else” or write “privacy” in your message); we respond by email or phone, as you prefer.
What we collect, and why
Website enquiries and callback requests. If you use the contact form (enquiry or callback), we collect what you give us: your name, who you are enquiring as, how you would like us to reply, your phone number or email address, the suburb or area, the kind of support or reason, a preferred time to call and any message you include. We use it only to respond and to work out whether we can help. If you give an email address and tick the box, we send one short acknowledgement email; it confirms receipt and is not an agreement to provide support. The forms deliberately do not ask for diagnoses, NDIS numbers, care plans or medical records, and we ask you not to include them.
If we go on to provide support. We then collect the information needed to provide safe, appropriate support and to invoice for it, which can include health and disability information, NDIS plan details and emergency contacts. That information is collected directly from you or your nominee, or from professionals you have agreed we can talk to, and it is handled under the written service agreement.
This website. The website does not use advertising cookies or tracking pixels, and we do not currently run analytics; if privacy-respecting analytics are added later, this page will say what is collected and how to opt out. The site search runs entirely in your browser against an index built from this website; your searches are not sent to us or to any AI service. The web host may keep standard server logs (such as IP address and pages requested) for security and reliability.
How website enquiries travel
Form submissions are delivered to our business mailbox by an email delivery service used by this site (Resend) and are not stored in a database by the website; the hosting platform may keep short-lived technical logs. If you ask for an acknowledgement, it is sent from a sending address on our domain that is not monitored; the contact form is the way to reach us. We call you back from a business mobile; outbound calls may show a caller ID.
Who we share information with
Only with the people and organisations you have agreed to: for example your plan manager (for invoicing), your support coordinator or the clinicians whose plans we follow. We may also disclose information where the law requires it, including to the NDIS Quality and Safeguards Commission in relation to a reportable incident, or where it is necessary to prevent a serious threat to someone’s life, health or safety. We do not sell or trade personal information.
How we keep it secure
Records are kept in password-protected systems with access limited to the people in the business who need them. We keep information only as long as it is needed for the purpose it was collected for and for any period required by law, and then delete or securely destroy it.
Your rights
You can ask what personal information we hold about you, ask for it to be corrected, and ask us to delete information we no longer need. You can also ask how we have handled it. Contact us using the details above. If you are not satisfied with our response, you can complain to the Office of the Australian Information Commissioner (OAIC). Complaints about NDIS supports can also go to the NDIS Quality and Safeguards Commission, which handles complaints about any NDIS provider.
Children and young people
Enquiries about a child are usually made by a parent, guardian or carer. We collect information about a child only for the purpose of assessing and providing support, with the consent of the person responsible for them.
Changes
If we change how we handle personal information, we will update this page and the date at the top.
