Support that fits around real life.
Everything here is one-to-one. Each support is shaped around the person, their routines and their goals, and what’s available depends on needs, location, our capacity and what we’re trained and permitted to do.
Community access and social participation
Getting out, getting to things, and being part of community life.
Practical one-to-one assistance with outings, appointments, groups, activities and everyday participation, at your pace and towards the things you want to be doing more of.
What’s included
- Getting to a group, class, sport or church
- Shopping, errands and appointments
- Seeing friends, the library, the gym or the park
- Building confidence on buses and trains
Who it suits, and limits
Suits: Anyone who wants regular, reliable company and practical help to get out and about.
Personal care and everyday routines
Respectful assistance with personal care, done your way.
Help with showering, dressing, grooming, toileting, mobility and personal routines, within assessed needs and our competence. Privacy, consultation and consistency matter most here, which is why we aim for the same worker and the same routine.
What’s included
- Morning and evening routines
- Showering, dressing and grooming
- Mobility, transfers and positioning within our training
- Keeping routines predictable
Who it suits, and limits
Suits: People who need regular personal care from someone they know and trust.
Hoist-assisted transfers
Within personal care and mobility, where it has been assessed as safe.
When hoist transfers can be included
We can assist with hoist transfers where the person’s care or transfer plan calls for it. This depends on an individual assessment, the right hoist and sling, safe manual-handling procedures, demonstrated competence with that equipment and that person, and a second worker where the plan or the task requires one. We don’t promise single-worker hoisting as a universal option. Hoist use on its own doesn’t make a support high-intensity under the NDIS schedule.
Suits: People with an existing transfer plan who want it followed carefully and consistently.
In-home assistance
Practical help with meals, the household and a comfortable home.
Meal preparation, appropriate household tasks, laundry, everyday routines and practical home-based support. Sometimes it sits alongside personal support; sometimes it’s the main thing you need.
What’s included
- Planning and cooking meals
- Cleaning, laundry and tidying
- Keeping the weekly routine on track
- A safe, organised living space
Who it suits, and limits
Suits: People who want hands-on help at home from a familiar worker.
The NDIS schedule prices purely domestic visits (cleaning, laundry) differently from support-worker assistance. See pricing.
Daily living skills and non-clinical mentoring
Practising everyday skills by doing them together.
Practical support to practise routines, planning, meal preparation, budgeting, getting around and similar everyday tasks, with a steady person alongside until you can do more of it yourself. This is practical mentoring, not therapy or specialist training.
What’s included
- Cooking simple meals together
- Working out a weekly budget
- Planning the week and keeping appointments
- Learning a bus or train route
Who it suits, and limits
Suits: People who want to do more for themselves and would like steady, practical support to get there.
Not occupational therapy, psychological treatment or specialist training. Which part of a plan funds it depends on the plan; your plan manager can advise.
Agreed participant transport
Transport with you in the vehicle, as part of scheduled support.
Where it’s part of agreed support, we drive you to and from the activity or appointment and stay with you where that’s the plan. It isn’t a taxi service, and we don’t charge for our own travel to or from you.
What’s included
- Travel to community activities and groups
- Appointments and essential outings
- A worker who stays with you where that’s part of the support
No fee for our travel to you.
We don’t charge for travelling to or from your location when you aren’t with us. If we transport you during agreed support, the current participant-transport rate is $0.99 per kilometre while you’re travelling with us. The relevant hourly rate still applies to the time we’re supporting you. We discuss and agree charges before support begins.
Who it suits, and limits
Suits: People who need a reliable way to get to the things their support is about.
Higher-intensity personal care, within verified competence
Complex daily personal activities, only where training, plan and safe staffing line up.
Some daily personal activities are classed by the NDIS as high-intensity, where the worker must hold specific skills under the NDIS Practice Standards. We’ll consider this work only where the particular tasks fall within current demonstrated competence, the person’s plan, safe staffing and the applicable practice requirements.
Scope and limits
Suits: Participants whose plan identifies high-intensity daily personal activities and who want a consistent worker.
Before any named procedure is agreed we confirm the skills descriptor, training, equipment, supervision and provider scope. Ordinary personal care is priced as standard support; only a support that genuinely meets the schedule’s high-intensity definition uses the high-intensity rate.
NDIS Practice Standards: high-intensity daily personal activities
Day-to-day use of an existing behaviour support plan
Everyday support that follows the strategies a qualified practitioner has already written.
If a behaviour support practitioner has written a plan, we can provide everyday support that follows its non-restrictive, lawful day-to-day strategies, with proper coordination and worker instruction, and in communication with the practitioner and your team.
Scope and limits
Suits: Participants with an existing plan who want their day-to-day support consistent with it.
This is not specialist behaviour support, assessment, plan writing or therapy. As an unregistered provider, Hope Filled Horizons does not implement regulated restrictive practices; if a plan includes one, that part needs a registered implementing provider, and we’ll say so up front.
We can
- Follow the positive, proactive strategies in your existing plan
- Keep records and communicate with your practitioner and team
- Be consistent, which is often what a plan needs most
We don’t
- Assess behaviour or write plans
- Provide specialist behaviour support
- Claim to be a registered specialist provider
Never while unregistered
- Implement any regulated restrictive practice
- Imply we can carry out a part of a plan that needs a registered implementing provider
What we don’t offer
- Counselling, psychology or allied-health therapy
- Specialist behaviour support: assessment, plan writing, or regulated restrictive practices
- Supports that only a registered provider may deliver, or NDIA-managed funding (see funding)
- A 24-hour or emergency service
Not sure whether what you need fits? Ask. We’ll tell you straight and, where we can, suggest who might help.
